The Prevention Needs Assessment (PNA) Survey is also being administered to middle school students in grades 6 and 8.
As a parent you have the right to decline your child’s right to complete the survey. Please let the Middle School Office know in writing if you do not wish your son or daughter to participate in the study.
If you wish to view the survey, a blank copy is available for viewing in the Middle School office.
The survey is being conducted under contract with Bach Harrison LLC, 116 S 500 E, Salt Lake City, Utah, 84102, a nationally known research firm. If you have any questions regarding the study, please contact Cassie Forbus or Chris Paige at Mountain View Prevention Services at 315-376-2321.
10/5/2020
Dear Parents:
Mountain View Prevention Services has been conducting the Prevention Needs Assessment (PNA) survey since 2001. Your child's class (grade 10 & 12) has been selected to participate in the PNA survey sometime during the weeks of October 5th through October 16th, 2020.
This survey measures alcohol, tobacco, and drug use; the factors that place students at risk for problem behaviors, as well as violence within our schools and communities. This survey will also help us gauge the success of our current prevention and intervention attempts.
The survey is entirely anonymous and voluntary. The surveys will be completed online, electronically so there are no names associated with the survey. Your child has the option of leaving blank any question that they encounter during the survey. All results from the study will be presented only in a group summary form with data collected from all of Lewis County.
As a parent you have the right to decline your child’s right to complete the survey. Please let me know in writing only if you do not wish your son or daughter to participate in the study.
If you wish to view the survey, a blank copy is available for viewing in the Middle School and High School offices.
